Since 1866 blue flags with 1, 2, 3, 4, or 5 stars have been used by the US Navy to designate flag officers. Two stars have always indicated the grade of rear admiral. Now the grade is split between rear admirals of the lower half who display one star, and rear admirals of the upper half who wear two stars.
